The California Dept. of Corrections (and rehabilitation) has gotten their collective ass in a crack over giving the court that supervises their medical operation a load of horseshit from time to time. This came up when a couple of prison pshrinks hollered "whistle blower discrimination" when they told the court that the department was fudging the data.
An outside legal group has gone over the allegations that the department committed fraud upon the court. The investigation, headed up by a former U S. Attorney, concluded that there was no deliberate fraud, the bad info was put out because CDCr is run by a pack of grotesque incompetents.
The review found that, "CDCr has a broken system of care because information is not accurately reported upon and reliable commonsensical action has not been taken. I have documented that patients are not getting to appointments on schedule and in confidential spaces, that appropriate consultation is not occurring, and worse, appropriate medical decision making by psychiatric physicians has been overridden. "

That fits very closely with my personal recollection of the system.
